Before conducting a psycho-educational assessment or starting with a psychological training it is necessary that you fill in a ‘Background Questionnaire’, so that I receive more information about the academic, social-emotional, behavioral development of your child. I will ask you to complete a ‘Parental consent form’ so that I can carry out the assessment or training with your child. In addition, you and/or the school might be asked to complete extra academic and/or social-emotional and behavioral forms when more information is required.
